NAC Benefits: Liver, Lungs, and the Glutathione Pathway

N-Acetyl Cysteine is the most-studied glutathione precursor. Here's why doctors use it in ERs, and why daily supplementation matters.

N-Acetyl Cysteine (NAC) is one of the few supplements you'll find in both your local health-food store AND the emergency room. ERs use intravenous NAC as the antidote for acetaminophen overdose, it works by rapidly restoring glutathione, the body's master antioxidant. The same mechanism, at lower oral doses, makes daily NAC one of the most useful supplements for liver and respiratory health.

What NAC actually does

NAC is a stable, bioavailable form of the amino acid L-cysteine. Cysteine is the rate-limiting nutrient for the synthesis of glutathione, the tripeptide your liver uses to neutralise toxins, your lungs use to clear mucus, and every cell uses to manage oxidative stress.

When you supplement NAC, your cells convert it directly into cysteine, which then combines with glutamate and glycine to form glutathione. Most other forms of cysteine are degraded too quickly or are poorly absorbed.

Liver support

The liver has the highest glutathione concentration of any organ. It uses glutathione constantly to:

  • Detoxify alcohol via the CYP2E1 pathway
  • Process common medications (acetaminophen, statins, NSAIDs)
  • Neutralise environmental toxins (pesticides, BPA, heavy metals)
  • Repair oxidative damage to hepatocytes

NAC supplementation has been studied for non-alcoholic fatty liver disease, mild hepatitis support, and recovery from heavy drinking. Standard dose: 600-1,200 mg per day on an empty stomach.

Respiratory and mucus support

NAC is mucolytic, it breaks down the disulfide bonds in mucus, making it thinner and easier to clear. Decades of European research support its use for:

  • Chronic bronchitis / COPD (reduces exacerbations)
  • Daily smokers' coughing
  • Recovery from upper respiratory infections
  • Allergy-induced congestion

During cold and flu season, many practitioners increase NAC to 1,200-1,800 mg per day split into two doses.

Other documented uses

  • Mental health, randomised trials have shown benefits for trichotillomania, skin-picking, and obsessive-compulsive symptoms at 1,200-2,400 mg/day.
  • Female fertility, combined with folate, NAC may improve ovulation in PCOS.
  • Exercise recovery, reduces post-workout oxidative stress, though may slightly blunt exercise-induced adaptations if taken right before training.

How to take NAC

  • Standard daily dose: 600 mg (1 capsule) on an empty stomach
  • Higher therapeutic doses: 600 mg twice daily
  • Best absorbed away from food
  • May cause mild GI upset in some people, take with a small snack if so

Safety + cautions

NAC has 50+ years of clinical use and an excellent safety profile. Cautions:

  • Avoid during pregnancy without a clinician's guidance
  • Can produce a sulfur smell in urine (harmless)
  • May reduce the effectiveness of nitroglycerin or some blood-thinning medications
  • Long-term doses above 3,000 mg/day may deplete zinc and copper, supplement separately if going high-dose
