Probiotic Strains: Which Ones Actually Matter for You?

Probiotics aren't generic, different strains do different things. Here's a strain-by-strain cheat sheet.

Most probiotic labels say things like 'supports digestion' or 'boosts immunity', vague claims meant to apply to any blend. But probiotics aren't interchangeable. Each strain has its own clinical evidence for specific outcomes. Here's the strain-by-strain reality.

Why strain specificity matters

Saying 'Lactobacillus' is like saying 'mammals', wildly imprecise. The species Lactobacillus rhamnosus is different from Lactobacillus reuteri, which is different from Lactobacillus plantarum, and within rhamnosus, the GG strain is different from the LRH-87 strain. Each has been tested for specific outcomes.

Top strains by outcome

Antibiotic-associated diarrhea

  • Saccharomyces boulardii, best evidence; resists antibiotics so it survives during treatment
  • L. rhamnosus GG, meta-analyses confirm reduced AAD risk

IBS

  • Bifidobacterium infantis 35624 (Align), strongest single-strain evidence
  • L. plantarum 299v, IBS bloating reduction
  • VSL#3 multi-strain blend, strong evidence in ulcerative colitis and pouchitis

Mood (the gut-brain axis)

  • L. helveticus R0052 + B. longum R0175 (Cerebiome), multiple anxiety/depression trials
  • L. plantarum PS128, emerging mood evidence

Immune support

  • L. casei DN-114001 (Actimel/DanActive), reduced respiratory infection frequency
  • L. paracasei 8700:2, cold and flu studies
  • B. animalis subsp. lactis HN019, immune cell activity in older adults

Vaginal and urinary health

  • L. crispatus CTV-05, vaginal microbiome restoration
  • L. rhamnosus GR-1 + L. reuteri RC-14, recurrent UTI prevention

Infants and pediatrics

  • L. reuteri DSM 17938, colic in breastfed infants
  • L. rhamnosus GG, acute gastroenteritis

What dose actually matters

Look for CFU at expiry, not manufacture. 10-50 billion CFU is the typical effective range. Doses above 100 billion rarely have additional benefit, and may cause transient bloating.

Survival to the gut

Spore-forming species like Bacillus coagulans are exceptionally hardy. For Lactobacillus and Bifidobacterium, look for delayed-release capsules or enteric-coated formats to survive stomach acid.

Buy multi-strain blends for general use

If you don't have a specific complaint, a multi-strain blend (8+ strains, 25-50 billion CFU) provides broader microbiome support than any single strain. Garden of Life, Seed (DS-01), and Jarrow Formulas Jarro-Dophilus are well-validated.

Bottom line

Match the strain to your goal. For general use, a multi-strain blend. For specific conditions, look up which strains have been tested for that condition, and demand that exact strain on the label.
