Black cohosh (Actaea racemosa) is a North American plant traditionally used by Native Americans for women's health complaints. The standardized Remifemin extract (developed in Germany in 1956) is the form used in most clinical trials. Multiple meta-analyses confirm meaningful reductions in hot flashes, night sweats, mood disturbance, and sleep difficulty in perimenopausal and postmenopausal women. Unlike phytoestrogens, black cohosh does not bind estrogen receptors, its effects are mediated through serotonin and opioid pathways. Standard dose is 40 mg of standardized extract per day, on continuous use for at least 8 weeks.
